Empowering children, young people
and adults to maximise their potential
We are a team of highly experienced case managers and clinical practitioners. We have a fantastic multidisciplinary team with our case managers having backgrounds in occupational therapy, physiotherapy and speech and language therapy. We also have a registered nurse who oversees medication and complex care. This enables us to have an excellent provision of service ensuring we deliver case management and care that is safe, effective, caring, responsive and well led.
Our case managers’ experience and knowledge enables them to empower their clients to reach their potential and engage in activities which are meaningful to them during childhood, adulthood and older age.
Our ethos is to be innovative, adult and child friendly and think outside the box so that the client’s independence and quality of life is maximised.
Integral to our role is advocating for our clients, working closely with their family, promoting autonomy, quality of life and doing all we can to move forward rehabilitation. As part of this we set up and oversee employed packages of care in addition to agency care. We ensure that carers working with our clients are promoting their choices, rehabilitation, dignity and quality of life. We are CQC registered and we are passionate to ensure that our clients and their families have support which is safe, caring and promotes choice, progress in rehabilitation goals and quality of life.
We offer bespoke and flexible case management which is driven by the client’s needs and wishes. Case management can be long term, goal driven or short term crisis intervention.
We understand the impact of lifechanging injuries on the client and their family.
We are sensitive to the pressures and complexities that are encompassed in this and we promote collaborative working, placing the client and their family at the centre.
